WHAT is  eRequest?  Welcome to the City of Charlestown's easiest and most convenient way for its citizens to communicate requests for Charlestown City Services, such as from the Police, Water, Wastewater, Street, Sanitation or Parks & Recreation Departments.  The City Services eRequest is not designed for RUSH situations, such situations should immediately be directed to (812) 256-3422.  If calling after normal operating hours, follow the automated system directions to summons the on-call representative.  In the event of an EMERGENCY dial 911 to contact the Charlestown Police Department and Charlestown Emergency Services.

 

eRequest Form Directions:  Complete the necessary sections of the below electronic form and click SEND ONCE.  Your request will be automatically forwarded to City Services Administration at Charlestown City Hall.  City Services eRequests are received during normal business hours, and handled in priority order, pending availability of request.  ONE REQUEST PER FORM SUBMISSION PLEASE.
